After route intake, complete all sections of the Parent Consent & Medical Intake Form: student and guardian details, pickup and destination, communication preferences, sensory triggers and comfort items, physician, allergies, emergency hospital preference, consent, signature and date. Read and sign the Policies & Liability Disclosure. Facility partners also complete the B2B Service Agreement. Download, sign, scan and return the documents to the email above, or hand them to your transport team.
